From time to time, we may request personal information from you at our sites in order to deliver requested materials to you, respond to your questions, or deliver service. 

The only individuals who are authorized to have access to nonpublic personal information about Patients are those individuals who need it to perform their job responsibilities or to provide products or services to Patients. For example, we may access Patient Information to offer other compatible products or services we provide, to process requests we receive from a Patient and to administer our products or services. Our employees are required to maintain the confidentiality of Patient Information and to follow the policies and procedures we establish to secure such information. In addition, we maintain physical, electronic and procedural security measures to safeguard Patient Information.
